Start with the essentials.
The secure consultation form is provided by Google Forms and opens on a Google website.
The form asks for your name, optional business name, email address, optional phone number, service needed, a short description of the problem, and your preferred contact method.
Do not include confidential, medical, financial, regulated, or personally sensitive information. Information submitted through Google Forms is handled according to the Brightpath privacy notice.
